Job summary

Stonecreek Health and Rehabilitation is seeking a qualified Occupational Therapy Assistant to provide therapy services under the direction of a licensed OT. You will participate in implementing individualized treatment plans and support residents in activities of daily living.

Collaborate with an interdisciplinary team, document accurately, and maintain confidentiality while delivering high-quality care in a supportive environment that values growth and resident-focused outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Valid state Occupational Therapy Assistant license.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Provide occupational therapy and related services under the direction of a licensed OT.
• Implement treatment for assigned patients in accordance with the established plan of care.
• Comply with state and federal guidelines regarding documentation and billing.
• Establish and maintain effective patient-centered interactions with patients and staff.
• Provide direct supervision to rehabilitation technicians during group and individual treatment.
• Participate in utilization review, case management, and care conferences as directed by the OT.
• Maintain awareness of changes in patient conditions and communicate progress timely.
• Maintain confidentiality of organizational, facility, and patient information.
• Conduct oneself in an ethical, legal, and professional manner.
