Job summary

Stonecreek Health and Rehabilitation is seeking a qualified Occupational Therapy Assistant to provide services under the supervision of a licensed occupational therapist. You will support individualized treatment plans and collaborate with an interdisciplinary team to help residents regain independence and improve daily living skills.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Valid state Occupational Therapy Assistant license.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Assist in screening and providing input to initial evaluations as appropriate.
• Implement treatment for assigned patients in accordance with the established plan of care.
• Comply with state and federal guidelines regarding accurate and timely documentation and billing.
• Establish and maintain effective patient-centered interactions with patients, interdisciplinary team members, and facility staff.
• Provide direct supervision to rehabilitation technicians during group and individual treatment.
• Participate in utilization review, case management, care conferences, and administrative meetings as directed by the occupational therapist.
• Maintain awareness of changes in patient conditions and programs and communicate patient progress in a timely manner.
• Maintain confidentiality of organizational, facility, and patient information.
• Conduct oneself in an ethical, legal, and professional manner at all times. Perform other duties and responsibilities as assigned.
