Job summary

Harvest-Crossing-Post-Acute in California is seeking a Certified Occupational Therapy Assistant to support our rehab team under the supervision of licensed therapists. You will implement treatment plans, document patient progress, and contribute to interprofessional care while maintaining compassionate, patient-centered service.

This role emphasizes accurate charting, ongoing professional development, and collaboration with therapists, nursing staff, and families in a busy long-term care setting.

Qualifications

• Graduate of an approved Occupational Therapy Assistant curriculum.
• Current, active license as Physical Therapist, and able to practice unencumbered.
• CPR Certification required for license renewal.
• Experience in long term care preferred.
• Must read, analyze and interpret information in English.

Responsibilities

• Implements and executes treatment plans under supervision of a therapist.
• Communicates with patients, families, physicians and care team.
• Provides timely documentation per facility requirements.
• Maintains professional skills and participates in education.
• Coordinates with team on treatment goals and changes in patient status.
• Attends meetings designated by Clinical Coordinator and provides feedback.
