Job summary

Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Lawrenceville, NJ. This PRN position supports resident care by delivering therapy under supervision to ensure high-quality outcomes.

Responsibilities include screening residents, implementing treatment plans, documenting care, and collaborating with the interdisciplinary team to meet CMS standards and ensure accurate billing.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans under supervision and follow goals.

• Establish resident therapy schedules with team members.
• Ensure documentation and billing of OT services following plan of care.
• Adhere to CMS documentation standards daily.
