Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Hammonton, NJ. The PRN position supports therapy services, focusing on resident-centered interventions and treatment planning.

Qualifications

• Must hold active OTA licensure in state(s) of practice
• Assoc. degree required in Occupational Therapy
• Ongoing continuing education and professional development to maintain licensure

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services in facility per standards
• Screen residents to determine best intervention per scope of practice
• Implement treatment plans and document progress and billing
• Coordinate therapy schedules with team members
• Communicate evaluation findings and recommendations to residents and families
• Maintain CMS documentation standards daily
