Job summary

Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Jellico, TN. This PRN position focuses on delivering high-quality occupational therapy services aligned with a physician-ordered plan of care in a facility setting.

The OTA will screen residents, implement treatment plans, document findings, and collaborate with the interdisciplinary team to ensure effective care while maintaining CMS-compliant documentation and billing

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine best intervention or treatment.
• Perform treatment plans for residents following established plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with team members.
• Document and bill occupational therapy services per plan of care.

Education
