Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Occupational Therapist (OT) for a full-time role in the Baltimore area, providing home health visits from the Towson, MD office.

The OT will serve adult and geriatric patients across Baltimore City and surrounding areas, joining a multidisciplinary team to deliver rehabilitative care in patients’ homes. The role emphasizes thorough assessments, care plan development, safety evaluations, patient education, and accurate documentation with EMR

Qualifications

• Current Maryland Occupational Therapist license.
• Prior home care experience preferred, but not required.
• At least one year of OT experience.
• OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Revise care plan with nursing in multidisciplinary team.
• Educate patients and family members in rehabilitative care to promote safety and independence.
• Document observations and interventions using a touch pad tablet.
