Job summary

GEM Transition Care Center is seeking a Certified Occupational Therapist Assistant to join the rehabilitation team in our 75-bed skilled nursing facility. You will work under the supervision of the Occupational Therapist and Rehabilitation Director to administer therapy plans and monitor patient progress.

The role emphasizes hands-on care, adherence to documentation standards, and collaboration with therapists, patients, and families to achieve recovery goals in a long-term care setting.

Qualifications

• CPR certification is mandatory.
• Licensed COTA is required.
• Preferably 1–2 years of experience in long-term care.
• Good physical and mental health is essential.
• Capable of lifting up to 50 pounds unassisted.
• Ability to follow both oral and written instructions.
• Possess training, knowledge, and experience for general duties under supervision.

Responsibilities

• Understanding of physical medicine, therapy modalities, anatomy, and physiology.
• Familiarity with operating physical therapy equipment, maintenance, and repairs.
• Administering treatment and rehabilitation services to patients with injuries or disabilities.
• Following treatment plans directed by the Occupational Therapist.
• Monitoring and documenting patient treatment and progress.
• Assisting in developing educational plans based on learning needs.
• Ensuring documentation meets current standards and policies.
• Adhering to facility's policies and procedures.
