Coordinated Therapy Solutions seeks a Certified Occupational Therapy Assistant to support licensed therapists in delivering occupational therapy services in a skilled nursing facility. The role emphasizes direct patient care, documentation, and collaboration with a care team to support progress and independence.

You will assess living skills, teach home programs, and assist with adaptive equipment while adhering to state and federal regulations. This position requires AS degree and licensure.

Qualifications

• Associate’s degree from an accredited occupational therapy program.
• Current state licensure as Occupational Therapy Assistant.
• Proficiency with all applicable state, federal, and professional standards.

Responsibilities

• Select therapy activities to fit patients’ needs and capabilities; observe progress and record in client records.
• Communicate and collaborate with other health care professionals involved in care.
• Instruct patients and families in home programs and adaptive equipment; help implement treatment plans.
• Assess daily living skills and assist patients in dressing and grooming.
• Assemble, clean, and maintain equipment for patient use.
• Attend care plan meetings to review progress and update plans.
