Job summary

Mount Sinai Health System is seeking a Per Diem Clinical Oncology Nurse Navigator to provide individualized support to patients, families, and caregivers. The role focuses on informed decision making, timely access to care, and coordination of follow-up visits along the cancer continuum within a team-based setting.

The ONN will collaborate with NPCs, clinical staff, and leadership to implement robust care plans, empower patients, and help overcome barriers across the care continuum while

Qualifications

• BSN required, with NY RN license.
• MS degree preferred.
• 3–5 years of nursing practice in oncology.
• 2–3 years oncology experience.
• ONS/ONCC Chemo Bio Cert preferred.

Responsibilities

• Delivers patient care by applying the nursing process and evidence-based standards.
• Organizes day-to-day patient care coordination.
• Assists patients in accessing cancer care and navigating health systems.
• Identifies barriers to care and helps families find solutions.
• Educates patients and caregivers on cancer treatment and team roles.
• Collaborates with the interdisciplinary team on the plan of care.
