Job summary

Topaz Fiscal Services LLC seeks a Credentialed Occupational Therapist to evaluate referrals, plan and document treatment programs, and collaborate with physicians and the rehab team to coordinate care. You will assess functional abilities, establish goals, and tailor interventions to restore independence and quality of life.

Travel within the local area is required; modality certification is preferred. The role requires licensure in the practicing state and prior OT experience.

Qualifications

• Credentialed graduate of an accredited OT program and licensed in the practicing state.
• Prior OT experience preferred.
• Ability to lift 50 pounds and frequent lifting up to 20 pounds.
• Travel within local area for patient treatment.
• Modality certification preferred.

Responsibilities

• Review referrals and patient records to determine OT treatment required.
• Plan and prepare written treatment programs based on data evaluation.
• Collaborate with physicians and rehab team to integrate therapy into care.
• Record treatment, responses and progress for documentation.
• Support rehab director in planning OT services and staffing as needed.
• Conduct tests and evaluations of functional activities and motor skills.
• Plan programs for functional skills and daily living activities.
• Coordinate therapy with other rehabilitation activities.
• Maintain confidentiality and appropriate documentation for reimbursement.
• Participate in department meetings and policy development.
