Job summary

Topaz Fiscal Services is seeking a licensed Physical Therapist to plan and administer prescribed physical therapy treatments, restore function, and reduce disability for patients/residents. Responsibilities include evaluating referrals, planning treatment programs, coordinating with physicians, documenting progress, supervising staff, and conducting training.

The role requires a credentialed PT, ability to lift, travel locally, and strong communication with residents and facility staff.

Qualifications

• Credentialed graduate of an accredited Physical Therapy program and licensed in the state where practicing.
• Prior experience in physical therapy preferred.
• Ability to lift up to 50 pounds and frequently lift/carry objects up to 10 pounds.
• Travel within a local area as needed for patient treatment.
• Ability to work indoors with exposure to some disagreeable conditions.
• Effective communication with residents, other therapy staff, and facility staff.
• Adherence to Employee Handbook terms including safety, regulations, and drug-free workplace.

Responsibilities

• Review physician referrals and patient records to determine PT treatment.
• Plan and write treatment programs based on data.
• Collaborate with physicians and health practitioners to optimize care.
• Record patient treatment, responses, and progress.
• Supervise PT assistants and aides; may train students.
• Administer manual therapy and use various physical agents.
• Evaluate treatment effects and adjust programs accordingly.
• Instruct and motivate patients in non-manual exercises and functional activities.
• Maintain documentation and contribute to department meetings and policy updates.
