Job summary

Tender Care Home Health & Hospice is seeking an Occupational Therapist in Las Cruces, NM. You will implement standards of care for occupational therapy services and ensure alignment with the Service Agreement.

Responsibilities include evaluating patients, developing care plans, fabricating splints, instructing patients and families, and maintaining timely clinical documentation. Field visits and collaboration with the director and staff are essential.

Qualifications

• Graduate of an occupational therapy curriculum and licensed to practice.
• Experience in home health settings preferred.
• Ability to document care plans, progress notes, and communications.

Responsibilities

• Evaluate patient’s functional status and home environment to plan care.
• Develop and revise the plan of care to improve function.
• Fabricate splints and instruct on adaptive equipment use when needed.
• Instruct patient and family; collaborate with care team for coordinated care.
• Maintain and submit clinical notes and progress reports within 48 hours.
