Job summary

Tender Care Home Health & Hospice in Las Cruces, NM seeks a Certified Occupational Therapy Assistant to provide therapy under the supervision of an OT. You will evaluate, implement, and adjust patient treatment plans while coordinating with a multidisciplinary team to support daily living activities at home.

The role requires NM licensure, a 2-year OTA degree, and current CPR. Experience in home health is preferred, with opportunities for growth and a supportive team environment.

Qualifications

• Graduate of a 2-year college-level program approved by the American Occupational Therapy Association.
• Minimum of one year of clinical Occupational Therapy experience.
• Meet CMS Conditions of Participation requirements for Homecare Agency.
• Licensed to practice OT by the state licensing board.
• Current CPR license.

Responsibilities

• Under supervision of an occupational therapist, assists in evaluating patients referred to occupational therapy.
• Implements rehabilitation treatment programs per physician orders and OT plan of care.
• Coordinates treatment plans with supervising OT and other team members.
• Maintains clinical records and submits timely documentation of patient progress.
• Explains goals to families and trains them for carry-over at home.
