Tender Care Home Health & Hospice in Las Cruces, NM, seeks a skilled Occupational Therapist (FT or PRN) to implement standards of care, evaluate patients, and develop comprehensive plans of care aligned with agency policies. You will coordinate with physicians, family members, and other staff to maximize patient independence and safety in home settings.

Responsibilities include patient evaluation, treatment planning, documentation within 48 hours, and supervision of OT aides.

Qualificações

• Graduate of an occupational therapy curriculum accredited by the AMA/AOTA.
• CPR certification required.
• Two years of clinical OT experience preferred.
• Good verbal and written communication and organizational skills.

Responsabilidades

• Evaluates patient’s functional status and home environment.
• Develops total plan of care and may fabricate splints.
• Instructs in use of adaptive and orthotic devices.
• Documents evaluations, plan of care, notes and progress reports within 48 hours.
• Supervises OT assistants and home health aides every 2–4 weeks as applicable.
• Maintains clinical records and coordinates with care team.
• Participates in in-services, meetings, and ongoing professional development.
• Complies with HIPAA, infection control, and agency policies.
