Job summary

Compassionate Care Home Health, Hospice, and In Home Care is seeking a licensed Occupational Therapist for Merced County. You will implement care standards for OT services, assess patients’ functional abilities, and develop effective treatment plans in a home health setting.

The position emphasizes collaboration with physicians, caregivers, and the care team to ensure quality, compassionate care and safe discharge planning. Experience in home health and strong communication are valued.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/AOTA.
• Registered with NBCOT (OTR).
• Two years of clinical OT experience.

Responsibilities

• Evaluate patient functional status and home environment; identify needs and plan care.
• Develop and supervise treatment plans; communicate with physicians and care team.
• Instruct patients and families; fabricate splints and use of adaptive equipment when needed.
• Supervise OT assistants and ensure proper documentation and discharge planning.
