Job summary

Compassionate Care Home Health Agency in California is seeking a licensed Occupational Therapist to implement standards of care for home health OT services. You will work under the Organization's policies and perform direct patient care as defined by state practice act.

You will evaluate patients, develop treatment plans, coordinate with physicians, supervise aides, participate in discharge planning, and contribute to a compassionate, growth-oriented team.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the American Medical Association and the American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication, and organization skills.
• Possess and maintain CPR certification.

Responsibilities

• Evaluates patient functional status and home environment for barriers to independence.
• Identifies equipment needs and participates in the total plan of care.
• Develops a treatment program and sets goals for improved function; communicates plan to physicians and team.
• Instructs patients and families in treatments and use of adaptive equipment as appropriate.
• Supervises certified occupational therapy assistants per policy and state regulations.
• Participates in discharge planning and coordinates with home health staff.
