Responsibilities

• Assess patients by interviewing and reviewing personal history.
• Plan, coordinate, manage and implement support packages for residents.
• Develop and execute individual care plans and assist with Medicaid/Medicare applications.
• Assist residents with benefits applications (Food Stamps, housing) as needed.
• Maintain accurate patient documentation.
• Collaborate with multidisciplinary teams and act as a key worker.
