Job summary

The Social Worker at Clove Lakes Health Care and Rehabilitation Center in Staten Island provides assessment, counseling and education to residents and families, aligning with facility policies to meet medical, emotional and social needs on an individual basis.

You will develop care plans, assist with Medicaid/Medicare applications, coordinate services, and maintain thorough, confidential documentation while collaborating with a multidisciplinary team to support residents' dignity and well-being.

Qualifications

• Nursing Home Experience Required
• Masters or Bachelors Degree

Responsibilities

• Assess residents' needs by interviewing and reviewing personal history.
• Plan, coordinate, manage and implement support packages to help residents deal with difficulties and overcome dependencies.
• Develop and execute individual treatment plans, include Medicaid and Medicare applications for the patient.
• Assist residents with Food Stamps, Housing applications and phone assistance.
• Interview service users and assess current condition, needs, strengths and weaknesses.
• Maintain accurate patient documentation and ensure confidentiality.

Education
