Job summary

Atlantic Health is seeking a Nurse Practitioner for Primary Care at Linden in Union, NJ. You will collaborate with physicians and APPs to evaluate patients, plan follow-ups, and provide care, education, and medical diagnoses.

You will assist with chronic disease management and participate in departmental initiatives to improve procedures. Responsibilities include performing procedures, prescribing medications, and guiding treatment plans with physicians.

Qualifications

• Graduated from an accredited Nurse Practitioner program.
• Active License in the State of New Jersey.
• Experience preferred; new grads are welcome to apply.
• Strong work ethic and a team player who can work independently.

Responsibilities

• Obtains medical records, evaluates patients, orders tests, diagnoses diseases, prescribes medications and supervises treatments.
• Assists patients with chronic disease management and promotes health improvement.
• Guides and provides direction, education, and training.
• Performs medical procedures, such as injections, immunizations, suturing and wound care.
• Partners with physicians in developing treatment plans and advises changes in a patient's condition.
• Acts as a department representative to hospital committees or events.
• Performs other related duties as assigned.
