Job summary

Regions Hospital seeks a Program Specialist for DayBridge Mental Health to support the PHP team under LICSW supervision. You will assess psychosocial needs, plan treatments, and coordinate care for adults with mental health and substance use concerns.

Your role includes crisis de-escalation, safety planning, and educating families, while collaborating with an interdisciplinary team to ensure comprehensive, patient-centered care.

Qualifications

• MSW from CSWE-, CASSW-, or equivalent accredited program.
• Under supervision of LICSW, perform psychosocial assessments and develop treatment plans.

Responsibilities

• Provide psychosocial assessments and treatment planning under LICSW supervision.
• Monitor and support clients daily in PHP milieu, including safety planning and crisis de-escalation.
• Document services accurately in EMR and coordinate with care team.
• Communicate with third-party payers for authorization as needed.
• Educate clients and families about care plans and available community resources.
