Job summary

Telecare's Tacoma-based Community Alternatives Team (T-CAT) is seeking a dedicated Case Manager II to support individuals with mental illness in Pierce County, helping them live as independently as possible in their chosen communities.

Under supervision, you will assess needs, coordinate services, drive clients to appointments, and document progress while maintaining compassion, privacy, and professional boundaries.

Qualifications

• High School Graduation or GED equivalent with 2 years direct services in mental health or community services; or an associate degree with 1 year experience; or a BA in Social Services.
• Valid driver’s license and personal vehicle insurance, name listed as driver.
• Willingness to use personal and company vehicles to transport clients to appointments and meetings.

Responsibilities

• Under direct supervision, serves as primary support and case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
• Coordinate services, document progress, and support treatment planning.
