Job summary

Telecare is seeking a Case Manager-Specialist in Mental Health to support individuals with serious mental illness, helping them live independently within their communities. This full-time on-site role requires driving clients to appointments and coordinating with the treatment team.

Qualifications

• High School Diploma or GED equivalent required.
• Associates or better in Social Services or higher.
• Bachelors or better in Social Services.
• Must have at least 1 year of experience in the specialty area (e.g. Benefits, Education, Employment).
• Valid driver's license and personal vehicle insurance with your name as driver.

Responsibilities

• Serve as primary case manager for individuals with mental illness, helping them live independently in their community.
• Provide knowledge to the treatment team in a specialty area.
• Under direct supervision, perform duties to support clients' goals and treatment plans.
• Coordinate transportation for clients using personal or company vehicle.
