Job summary

Stars Behavioral Health Group is seeking a Mental Health Case Manager/Facilitator in Reseda, CA. The role coordinates wraparound services, leads engagement and assessment, and develops safety plans with a multidisciplinary team.

The position emphasizes case management for youth and families, collaboration with community agencies, and opportunities for CEU training and professional supervision. On-site hours are Mon–Fri, 8 AM–5 PM.

Qualifications

• Facilitator I/II education requirements and related credentials.
• Experience in mental health services with high-risk populations (SED).
• Licensure or certifications as specified (CANS, CFT training).

Responsibilities

• Coordinate services across programs, engagement, assessment, and CFT.
• Develop Safety Crises Plan and Strengths Summary for youth/family.
• Provide case management and potential mental health services as needed.
• Act as liaison with external agencies and coordinate team participation.
