Job summary

miOttawa.org is seeking a full-time Mental Health Case Manager in Grand Haven, MI. Under supervision, you will coordinate therapeutic treatments and community supports for individuals with mental illness and co-occurring disorders.

The role requires a Bachelor’s in human services and a QMHP credential; CADC preferred. Travel across the county is expected, with standard weekday hours at our Fulton Street office.

Qualifications

• Bachelor’s degree in a human services field and one year of experience is required; two years preferred.
• QMHP qualification is required; CADC certification strongly preferred.
• Valid Michigan driver’s license required.

Responsibilities

• Provide targeted case management and link clients to community resources and benefits.
• Coordinate care with primary care physicians and advocate within the medical community.
• Assist clients in housing, employment, and daily living skills; support self-sufficiency.
• Maintain case notes and ensure HIPAA-compliant documentation; prepare reports.
• Facilitate group services and participate in treatment teams to review progress.
