Job summary

You will perform ongoing assessments, coordinate care, and advocate for consumer needs within a team focused on adult mental health case coordination. Qualifications include a Bachelor’s (or Master’s) in social work and QMHP credentialing eligibility, plus 1 year of experience with chronic mental illness and related conditions.

Qualifications

• Bachelor’s degree in social work, clinical psychology, or counseling.
• Ability to be credentialed as a QMHP.
• One year experience with individuals with chronic mental illness, intellectual disability, and substance use disorders.
• Two years’ experience in a human services agency.

Responsibilities

• Provide ongoing assessment and person-centered planning.
• Coordinate care and linkage of resources; advocate for consumer needs.
• Facilitate the Person-Centered Planning process to ensure Medicaid compliance.
• Monitor utilization of community-based services and resources.
