Job summary

Momentum for Health in San Jose, CA is seeking a Case Manager to deliver mental health rehabilitation and case management services to a diverse caseload.

Responsibilities include developing treatment plans, coordinating care with families and outside professionals, documenting records, and assisting with community integration. A qualified candidate will have relevant education and experience in mental health services and be fluent in English; Spanish bilingual candidates are preferred.

Qualifications

• Progressive experience in mental health service settings.
• Eligibility for MHRS certificate.
• Ability to work independently and in small teams.

Responsibilities

• Provide mental health rehabilitation and case management for a varying caseload.
• Facilitate counseling processes to develop life skills.
• Educate families on rehabilitation processes for successful transitions.
• Coordinate care with outside mental health professionals and programs.
• Assist in budgeting and financial planning related to client services.
