Hope Services in Silicon Valley is seeking a Case Management Specialist for our Mental Health Services program. You will assess, plan and document services, connect clients with community resources, and advocate for outcomes aligned with Santa Clara County guidelines.

This role requires an AA degree or equivalent and experience with disabilities or rehabilitation. We offer comprehensive benefits and a supportive environment focused on wellness and recovery.

Qualifications

• AA degree or equivalent plus one year of related experience.
• Knowledge of substance abuse, mental illness, and developmental disabilities.
• Ability to observe, document and communicate verbally and in writing.
• Ability to develop plans and coordinate with providers.

Responsibilities

• Assist clients in obtaining and maintaining living situations.
• Provide case management on site and in the field.
• Coordinate with medical, dental and mental health providers.
• Advocate for clients with community resources and public benefits.
• Document notes and treatment plans in the electronic health record.
