Job summary

Boston Medical Center is seeking a Medical Case Manager for the Immigrant and Refugee Health Center in Boston, MA. This on-site, 40-hour-per-week role emphasizes biopsychosocial patient care and data-driven coordination across psychiatry, internal medicine, OB/GYN, and social services.

Strong interpersonal skills and trauma-informed approach are essential. The position requires a MSW or related degree, with LCSW/LICSW preferred, and 2–3 years of relevant experience.

Qualifications

• Masters’ degree in social work (MSW) or counseling preferred, or Bachelor’s degree in social work or related human services field.
• LCSW/LICSW preferred, but not required.
• Required Experience: 2-3 years of experience working in a medical or social services environment.
• Preference for bilingual candidates (English plus other languages).

Responsibilities

• Conducts needs assessments/intake for new patients, including mental health acuity and triage.
• Perform comprehensive assessment of medical, mental health and social needs.
• Coordinate scheduling and referrals across departments and services.
• Document needs assessment data in EMR and maintain HIPAA standards.
• Provide case management for a caseload of IRHC clients and connect to resources.
