Job summary

CAN Community Health Inc. is seeking a Medical Case Manager I in the Miami area. This full-time role supports patients affected by HIV, Hepatitis C, and other infectious diseases by coordinating comprehensive case management and care access.

Qualifications

• Bachelor’s degree in social work, Nursing, Public Health, or related field.
• One year of HIV prevention/intervention experience.
• Please refer to state requirements per jurisdiction.
• Must be able to operate a motor vehicle and have valid insurance and driver’s license.

Responsibilities

• Coordinate and deliver comprehensive case management services to clients in Miami Gardens/Miami South Beach.
• Screen clients for program eligibility and conduct biannual reassessments for continued enrollment.
• Verify insurance coverage and coordinate required pre-authorizations.
• Develop, implement, and update individualized care plans with patient goals.
• Act as liaison between patients and the care team to address identified needs.
• Assist in collaborating, developing, implementing, monitoring, and evaluating the case management process.
• Perform comprehensive assessments across health, mental health, social support, addiction, financial resources, benefits, legal, language/culture, and employment.
• Evaluate client medical acuity to establish care priorities and evidence-based standards of service delivery.
• Coordinate client access to primary medical care and attend client appointments as needed.
• Submit monthly billing documentation in line with grant standards.
• Document all client encounters within software within 48-72 hours.
