Job summary

CAN Community Health, Inc. is seeking a Medical Case Manager I to coordinate comprehensive case management for clients, ensuring access to care and adherence to guidelines.

This role includes eligibility screening, care planning, and collaboration with healthcare teams to improve outcomes. The position requires a Bachelor’s degree in a related field and at least one year of HIV prevention/intervention experience, with background checks and valid driver’s license.

Qualifications

• Bachelor's degree in social work, nursing, public health, or related field.
• One year of HIV prevention/intervention experience.
• Must be able to operate a motor vehicle and have valid insurance and driver’s license.
• Must be able to pass a Level I and Level II Background check as required.

Responsibilities

• Coordinate and deliver comprehensive case management services to clients in line with guidelines.
• Screen clients for program eligibility and biannual reassessments.
• Verify insurance coverage and coordinate required pre-authorizations.
• Develop and update individualized care plans with patient goals.
• Act as liaison between patients and care team to address needs.
• Assist in coordinating, implementing, monitoring, and evaluating case management processes.
• Conduct comprehensive assessments across health, mental health, social support, and other areas.
• Evaluate client medical acuity to set priorities and standards of service delivery.
• Coordinate access to primary medical care and attend appointments as needed.
• Submit monthly billing documentation per grant standards.
• Document all encounters within software within 48-72 hours.
