Chicago Skilled Nursing in Chicago, IL is seeking an experienced MDS Coordinator to join our team at our skilled nursing facility. You will develop, monitor, and modify care plans, perform resident assessments, and assist in the discharge process to ensure high-quality care.
Applicants must hold an Illinois RN license, CPR certification, and have at least two years of experience as an MDS Coordinator, plus two years as an RN in an SNF and one year as a CNA.
Chicago Skilled Nursing has an opportunity for an MDS Coordinator to join our team at our Chicago Skilled Nursing Facility.
As the MDS Coordinator, you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process.
To be eligible for consideration applicants must have:
As a minimum, an unencumbered State ofIllinois R.N. License, be a graduate of an accredited nursing program;
C.P.R. Certification;
at least two (2) years of prior experience as a MDS Coordinator;
two (2) years of experience as an R.N. in a Skilled Nursing Facility setting;
and (1) year of experience as a C.N.A..
