Job summary

Longterm Health Management Services is seeking an MDS Nurse/Coordinator to join our skilled nursing team in Illinois. The role focuses on developing and monitoring individualized care plans and ensuring high-quality resident care.

The candidate should hold an Illinois RN license, be a graduate of an accredited nursing program, and possess CPR certification. Prior MDS Coordinator experience and SNF RN/CNA experience are preferred to support our residents’ needs.

Qualifications

• As a minimum, an Illinois RN license and graduation from an accredited nursing program are required.
• CPR certification is required.
• Prior experience as a MDS Coordinator is preferred.
• One year of RN experience in a skilled nursing facility is required and one year of CNA experience is preferable.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan for individual needs and goals.
• Perform resident assessments and assist in the discharge process.

As a MDS Nurse you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at our facility by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State ofIllinoisR.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable!
