Qualifications

• Active New Hampshire LPN license.
• Experience in facility, long-term care, rehabilitation or similar settings preferred.
• Strong clinical judgment and attention to detail.
• Dependable, professional, and compassionate approach to patient care.
• Excellent communication and teamwork skills.
• Ability to adapt quickly to different facility environments.
• Comfortable working independently while collaborating with the care team.

Responsibilities

• Provide skilled nursing care to patients and residents in a facility setting.
• Administer medications and treatments as ordered within scope of practice
• Monitor patients for changes in condition and communicate concerns to the appropriate members of the care team
• Complete accurate and timely clinical documentation
• Assist with assessments and contribute to individualized plans of care
• Communicate effectively with RNs, LNAs, providers, patients, residents, and families
• Promote patient safety, comfort, dignity, and quality of care
• Follow facility policies, procedures, and applicable nursing standards
