Job summary

Amazing Care Home Health Services is seeking an Occupational Therapist to provide in-home, individualized therapy to adult clients in Grand Junction, CO. You will help patients improve independence, mobility, and overall quality of life through skilled, client-centered care.

We offer competitive pay per routine visit ($70.00 - $85.00) and SOC visits up to $120, with part-time availability (10-20 visits/week) and a flexible schedule designed to support work-life balance and professional growth.

Qualifications

• Valid OT license in Colorado.
• Proven experience in home health or related field.
• Excellent communication and interpersonal skills.
• Proficiency in electronic medical record documentation.
• Flexible and adaptive approach to meet diverse client needs.

Responsibilities

• Develop and implement personalized treatment plans.
• Conduct evaluations using standardized tools and clinical expertise.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Educate patients and caregivers on home programs and adaptive equipment.
• Maintain timely and accurate clinical documentation.
• Collaborate with a multidisciplinary care team to ensure coordinated support.
