Job summary

Qualifications

• Must hold at least a Bachelor’s degree in Social Work or a related field from an accredited university.
• Experience with child welfare, early childhood education, or children’s mental health is preferred.
• Must become a certified Wraparound Facilitator in high‑fidelity Wraparound within 3 months of employment.

Responsibilities

• Provide Case Management services under the Family Works program.
• Assess, link, coordinate, and monitor services for families.
• Develop individualized treatment plans collaboratively with the Integrated Decision Team.
