Job summary

The Institute for Human Services (IHS) in Honolulu, Hawaii, is seeking a CIS Housing Navigator to provide pre and post tenancy services for homeless clients, coordinate access to housing programs, and maintain quality case management and crisis support. • Understanding insurer benefit provisions and public funding programs for clients.

Responsibilities

• Provide pre and post tenancy services for homeless clients and ensure necessary forms and consents are obtained.
• Link clients to community resources to access housing programs; coordinate with collateral contacts.
• Maintain client service plans and crisis support; conduct face-to-face contact at least twice monthly.
• Maintain the integrity of clinical records and ensure documentation meets IHS standards.
• Support development of transition/discharge plans for clients not meeting care criteria.
