Job summary

Wayne UNC Health Care is seeking a Pharmacy Medication History Technician (MHT) to obtain and document patients' current pre-admission medications into the hospital's electronic medical record. You will ensure accuracy for use by providers, nurses, and pharmacists during admission, discharge, and transitions of care.

Responsibilities include interviewing patients and families, verifying information with providers and pharmacies, documenting histories, and notifying pharmacists of discrepancies.

Qualifications

• High school diploma or equivalent required and completion of a certified pharmacy technician training program.
• Registration as a Pharmacy Technician with the North Carolina Board of Pharmacy within 30 days of employment.
• PTCB certification required and ability to obtain PTCB Medication History Certificate.

Responsibilities

• Interviews patients, family members, and guardians to obtain complete medication histories.
• Documents medication histories in the electronic medical record.
• Notifies pharmacists of discrepancies or safety issues identified in obtaining histories.
• Operates dispensing technology to supply medications per orders.
• Orders, restocks, and rotates inventory; manages expiration dates.
• Contributes to regulatory inspections and documentation.
