Corewell Health is seeking a Pharmacy Medication History Technician to obtain and document accurate medication histories for admitted patients. You will work under the pharmacist's direction, process histories, and clarify discrepancies with providers and caregivers.

The role emphasizes compliance with certifications, maintaining patient profiles, and collaborating with the pharmacy team in a busy hospital environment.

Qualifications

• Required High School Diploma or equivalent
• Preferred Associate's Degree health care or related field
• 1 year of relevant experience Pharmacy Required
• 2 years of relevant experience Retail Pharmacy Preferred
• LIC-Pharmacist Intern - STATE_MI State of Michigan Upon Hire required Or
• LIC-Pharmacy Technician - STATE_MI State of Michigan Upon Hire required And
• CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required Or
• CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required
• Ideal candidate will possess Full Pharmacy Technician License and National Certification, such as PTCB or ExCPT
• 1+ year of Pharmacy Technician experience required

Responsibilities

• Provide documentation and clarification of medication histories under the direction of the pharmacist.
• Process, enter, and interpret physician/nurse/pharmacist medication histories in the computerized system.
• Clarify information through communication with providers, patients, and pharmacies.
• Perform quality assurance data collection, collation, and presentation.
• Maintain patient profiles, verify allergies, duplicate meds, and last doses with the pharmacist.
• Maintain compliance with Corewell Health competencies and certifications.
• Maintain a clean and organized work area.
