Job summary

Tampa General Hospital seeks a Medication History Pharmacy Technician 1 to support pharmacists in ensuring accurate medication lists at admission and discharge. Under direct supervision, you interview patients, obtain external prescription histories, and document data in the EHR to support safe transitions of care.

You will update allergy histories, verify preferred pharmacies, compare lists across care settings, and escalate discrepancies to the supervising pharmacist, while following

Qualifications

• High School Diploma or GED.
• Registered Pharmacy Technician issued by Florida.

Responsibilities

• Obtain, update, and document an accurate medication list in the patient’s EHR at admission and discharge.
• Update patient allergy history and preferred pharmacy information in the EHR to support accurate medication reconciliation.
• Compare patient medication lists to identify differences between medications taken prior to admission, during hospitalization, and at discharge, and elevate discrepancies to the supervising pharmacist.
• Utilize all necessary sources - patient, family, hospital transfer orders, nursing and long-term care facility records, community pharmacies, and insurance fill history to ensure the medication list is complete and correct.
• Comply with all documentation requirements, including that for controlled substances and patient consultations, and follow supervisor notification procedures for dispensing incidents or PHI disclosures.
• Perform special precaution and PPE procedures, maintain infection-control standards, and complete all other pharmacy technician duties as assigned.
