Job summary

Compassus in Olympia, WA seeks a licensed Hospice Social Worker to support patients and families across home, long-term care, and hospital settings. You will assess psychosocial, financial, and environmental needs, provide crisis intervention, and collaborate with the interdisciplinary team to plan care and discharge.

Requires a Master of Social Work and WA social work licenses; 1 year post-master’s health care experience preferred.

Qualifications

• Master's degree from an accredited School of Social Work.
• 1 year post-Master’s experience in a health care setting (preferred).
• Washington Advanced Social Worker License (Vendor Managed) OR Washington Advanced Social Worker Associate License (Vendor Managed) OR Washington Clinical Independent Social Worker License (Vendor Managed) OR Washington Clinical Independent Social Worker Associate License (Vendor Managed)

Responsibilities

• Conducts clinical assessments of psychosocial, financial, environmental, and community needs and documents in care plans and notes.
• Delivers social services across homes, long-term care facilities, and hospitals; provides crisis intervention and counseling.
• Supports care planning, discharge planning, and coordination with the interdisciplinary team and external partners.
