Job summary

UnitedHealth Group is seeking a Home Health Registered Nurse (RN) for Decatur, AL. This full-time role involves providing skilled nursing care in patients' homes, conducting assessments, and coordinating with an interdisciplinary team to support positive patient outcomes.

You will work independently with a focus on quality and compliance, including on-call rotations and documentation duties. A valid Alabama RN license and reliable transportation are required, with CPR preferred and home health

Qualifications

• Current and unrestricted RN license in Alabama.
• Driver's license, vehicle insurance, and access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.
• 1+ years of home health experience.
• Ability to work independently and communicate clearly.

Responsibilities

• Complete nursing assessments and develop individualized plans of care.
• Provide skilled nursing care in patients' homes.
• Perform OASIS assessments and medication reconciliation.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Supervise home health aides and LPN/LVNs as appropriate.
• Document visits accurately and in accordance with payer and agency requirements.
• Communicate changes in patient condition and coordinate care needs.
• Support quality initiatives that improve patient outcomes and reduce avoidable hospitalizations.
• Participate in on-call and weekend rotations as needed.
