Job summary

Addus HomeCare is seeking a Registered Nurse to deliver professional nursing care in patients' homes in accordance with the plan of care and company policies. You will work with the care team to ensure high-quality, compassionate home health services.

Responsibilities include performing nursing interventions, documenting care in the medical record, coordinating with physicians and families, and guiding LPNs and aides.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a RN in the state of practice.
• At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing.
• Home health experience preferred.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to complete necessary documentation appropriately, legibly and timely.
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
• Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation.

Responsibilities

• Assess, plan and intervene using the nursing process to meet the patient's health care needs.
• Observe and report signs or symptoms indicative of changes in condition and notify supervisor.
• Perform nursing procedures, including administration of treatments and medications.
• Teach patients/caregivers appropriate self-care techniques.
• Notify Case Manager, physicians, and families of changes in condition.
• Supervise LPNs and Home Health Aides per state regulations and provide instructions.
• Maintain data collection, recording, and reporting systems for outcomes measurement.
• Communicate potential risk management issues and report abuse or unsafe practice.
• Attend multi-disciplinary conferences, staff meetings and in-service education.
• Coordinate patient care with patient, family and health care team.
• Participate in on-call schedule and after-hours visits.
