Job summary

Addus Family Company is seeking a compassionate Registered Nurse PRN to deliver in-home skilled nursing care to elderly and chronically ill patients. You’ll perform assessments, develop and update care plans, administer medications and treatments, and provide patient and family education.

Qualifications

• Active RN license in NM or multi-state
• Experience in in-home or chronic care preferred
• Ability to assess and develop comprehensive care plans
• Experience administering medications and treatments

Responsibilities

• Perform comprehensive in-home assessments and develop/update care plans
• Administer medications and treatments as prescribed
• Provide patient and family education on care plans and self-management
• Collaborate with interdisciplinary team to promote safety, comfort and independence at home
• Maintain compliance with regulatory standards and ensure quality of care
