Job summary

Addus HomeCare is seeking a dedicated registered nurse to deliver professional home health care, implementing the established plan of care and adhering to state/federal requirements. You will assess patients, administer treatments, educate families, and coordinate with the care team to ensure high-quality outcomes.

The role requires RN licensure in the practice state, prior home health or acute care experience, and strong documentation and communication skills.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a RN in the state of practice.
• Home health experience preferred.
• Clinical nursing experience in ICU, pediatrics, rehab or medical-surgical nursing preferred.
• Ability to perform documentation accurately and timely.
• Strong communication with patients, families and teammates.

Responsibilities

• Assess, plan and intervene using the nursing process to meet patient needs.
• Administer treatments and medications per plan of care and regulations.
• Teach patients/caregivers appropriate self-care techniques.
• Maintain data collection and reporting for outcomes and compliance.
• Coordinate home visits and communicate with Case Manager and physicians.
• Attend multi-disciplinary meetings and participate in on-call schedules.
