Job summary

Addus HomeCare is seeking a dedicated Registered Nurse to deliver professional home health nursing in patients' homes, following the established plan of care and company policies. You will ensure care aligns with state and federal requirements and maintain high standards of confidentiality.

Responsibilities include assessing and planning care, administering treatments, educating patients and caregivers, and coordinating with multi-disciplinary teams.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed RN in the state of practice.
• One year of clinical nursing experience preferred in ICU, rehab, or med-surg.

Responsibilities

• Assess and plan patient care using the nursing process.
• Deliver nursing care in accordance with plan of care and regulations.
• Administer treatments and medications per State Nurse Practice Act.
• Educate patients and caregivers on self-care techniques.
• Communicate changes in patient condition to Case Manager and physicians.
• Conduct supervisory visits for LPN and Home Health Aide where required.
• Maintain accurate data collection and reporting systems.
