Job summary

CenterWell Home Health is seeking a licensed Physical Therapist Assistant to join our patient-centered care team. You will travel to patients’ homes to carry out the treatment plan created by a Physical Therapist, helping them regain strength, mobility and independence.

Requirements include an active PT license, CPR certification, a valid driver’s license and English fluency. Prior home health or geriatric care experience is preferred. 1 year of PTA experience is a plus.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license with auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of geriatric physical, emotional, social and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Perform non-complex manual therapies and modalities to patients.
• Document client records and submit clinical documentation to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment issues, supply shortages, or staff changes.
• Apply infection control and universal precautions in patient care.
