Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver PT services in patients' homes, under supervision of a Physical Therapist. You will implement treatment plans, document care, and collaborate with a multidisciplinary team to support functional improvements for geriatric patients.

The role requires a PTA license, active CPR, a valid driver's license, and reliable transportation. A collaborative, patient-centered environment with 40-hour work weeks is provided.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, on-site in patients' homes.
• Administer non-complex active/passive exercises, massage, traction, heat/cold modalities.
• Document client records and submit weekly clinical documentation.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff changes.
• Apply infection control and universal precautions in patient care.
