Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver PT treatment in patients' homes under the supervision of a Physical Therapist. You will document care and support functional improvement to help patients regain independence.

Qualifications include an active Physical Therapy license, a valid driver’s license, CPR certification, and ability to work with a care team. The role requires English fluency and prior geriatric care exposure.

Qualifications

• Current state license to practice Physical Therapy.
• Read, write and speak fluent English.
• Experience collaborating with a healthcare team.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in patients' homes.
• Administer non-complex active and passive manual therapies, massage, heat/cold modalities as directed.
• Document client records and submit clinical documentation on time.
• Attend educational meetings and coordinate department in-services.
