Job summary

CenterWell Home Health is seeking a licensed Physical Therapy Assistant to deliver in-home physical therapy under the supervision of a Physical Therapist. You will implement treatment plans, conduct routine exercises, document progress, and coordinate with a clinical team to support geriatric patients' functional improvement.

The role requires current state PTA license, valid driver's license, CPR, and strong teamwork.

Qualifications

• Current state license to practice Physical Therapy (PTA).
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of geriatric physical, emotional, social and biological changes.
• Read, write and speak fluent English.
• One year of PTA experience (preferred).

Responsibilities

• Administer the physical therapy treatment plan under the PT, in patients' homes.
• Perform non-complex active and passive exercises, massage, heat/cold modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in care activities.
